Wound and Incision Surveillance After Vascular Procedures

Discharge instructions for procedures involving groin incisions must specify wound monitoring criteria that distinguish expected post-operative ecchymosis and swelling from early wound complication signs: increasing erythema extending beyond the immediate peri-incisional margin, induration, warmth, serous or purulent drainage, wound separation, or lymphorrhea. Patients with diabetes or obesity face elevated wound complication risk and may benefit from earlier scheduled wound checks.
- Hematoma and pseudoaneurysm surveillance instructions are required for patients who underwent open arterial access or percutaneous femoral access with arterial closure device use. A palpable pulsatile mass at a femoral access site developing within days to weeks of the procedure is the clinical presentation of a femoral pseudoaneurysm and requires duplex ultrasound evaluation. Discharge instructions should specify that any new pulsatile bulge or rapidly enlarging non-pulsatile mass at a groin or extremity access site warrants prompt provider contact for imaging evaluation. Discharge instructions should distinguish lymphorrhea from early wound infection, specify the wound drainage characteristics that indicate lymphorrhea versus infection, and instruct the patient that persistent clear drainage beyond 5 to 7 days without wound closure warrants provider contact. Untreated lymphorrhea can progress to wound dehiscence and serve as a portal for secondary infection.
- Incision monitoring after endovascular aortic repair (EVAR) via bilateral groin access requires attention to both access site wounds and to flank or abdominal discomfort that may indicate an expanding retroperitoneal hematoma or early endoleak-related aortic sac pressurization. Discharge instructions should specify that groin wound complications after EVAR are managed similarly to other groin access wounds, but that any new back pain, abdominal distension, or hemodynamic symptoms in the early post-operative period warrant emergency evaluation rather than outpatient scheduling.
Graft and Stent Surveillance and Limb Monitoring

- Access site and stent thrombosis monitoring applies to patients who underwent percutaneous arterial interventions with stent placement in peripheral arteries, renal arteries, mesenteric arteries, or the carotid. Antiplatelet therapy adherence is critical to maintaining stent patency, and discharge instructions must specify the prescribed antiplatelet regimen, the duration of dual antiplatelet therapy where applicable, and the importance of not discontinuing antiplatelet medications without consulting the vascular surgeon. Patients discharged on DOACs require instructions on dosing schedule adherence, the importance of not missing doses to maintain anticoagulant effect, and the need to notify all treating providers and pharmacies of the anticoagulant to prevent harmful drug interactions.
- Fluid status and renal function monitoring instructions are relevant for patients with chronic kidney disease, congestive heart failure, or contrast exposure during endovascular procedures.
